01Brazilian butt lift (BBL): buttock augmentation through lipofilling
Buttock lipofilling — also called BBL, liposculpture or fat remodelling — involves harvesting excess fat from other areas of the body (belly, thighs, saddlebags, hips) to re-inject it into the buttocks. It is an autograft: volume is added and the curves are reshaped with 100% natural tissue, without any implant. It restores fullness to buttocks felt to be too flat, too small, loose or sagging (after pregnancy or weight loss), while improving projection and the arch of the lower back.

02Who is the BBL for?
Buttock lipofilling requires a sufficient fat reserve in the harvesting areas: it suits slim to normal-weight people who have this fat, with a BMI within the normal range. Very slim people will not achieve a noticeable volume and are better suited to buttock implants; people who are significantly overweight are not good candidates. It is suitable for both men and women who want naturally fuller, more shapely buttocks.
03BBL or buttock implants: the advantages of lipofilling
Compared with implants, the Brazilian butt lift offers several advantages:
- 100% natural and autologous: your own fat, so no risk of rejection or capsular contracture as with a silicone implant;
- permanent result (the fat that « takes » stays), whereas implants have to be replaced every 15–20 years;
- no visible scar: only micro-incisions, compared with the several-centimetre scars of a buttock lift or implant placement;
- double body-contouring benefit: the liposuction of the harvesting areas slims the waist and hips — you obtain an « S-shaped » body contour.
On the other hand, the volume gain remains more modest than with an implant and depends on the available fat. For augmentation without surgery, HYAcorp hyaluronic acid injections are a gentle but temporary alternative.
04BBL and safety: a subcutaneous injection, never into the muscle
The Brazilian butt lift’s reputation for danger comes from a rare but serious risk of fat embolism, which occurred when fat was injected into the gluteal muscle, close to large veins. Current international recommendations now require fat to be injected exclusively into the subcutaneous plane, never into the muscle, using a blunt cannula — which has sharply reduced this risk. It is this safe technique that is practised here. A BBL is only safe in an operating theatre, with a qualified plastic surgeon: beware of low-cost « Colombian butt lift » offers abroad.

06How the procedure works
Before the procedure
The BBL requires two consultations. The surgeon assesses your anatomy: the amount of harvestable fat, tissue suppleness and skin quality, to judge feasibility and a realistic volume. They explain the procedure, the recovery and the complications, and check for any contraindication. Usual instructions: stop smoking and oral contraception 1 month before, no aspirin 10 days before, a stable weight in the months leading up to the procedure, and an anaesthesia consultation at least 48 hrs beforehand.
During the procedure
The procedure lasts 2 to 3 hours, under general anaesthesia, usually with one overnight stay (sometimes as a day case). It takes place in three stages:
- Harvesting: a liposuction of the hips, the flanks (love handles) and the saddlebags collects the fat through micro-incisions, while reshaping the small of the back.
- Processing: the fat is purified in a closed circuit (decanted, washed, centrifuged), cleared of blood and debris, so that only healthy fat cells are re-injected.
- Re-injection: the fat is re-injected gradually, with a blunt cannula, exclusively into the subcutaneous plane (never into the muscle), to optimise graft take and distribute the volume evenly.
The micro-incisions are closed with absorbable sutures and a shaping dressing is applied.

After the procedure
Recovery is moderate: swelling and bruising, some pain in the harvesting areas (eased by painkillers). An anticoagulant treatment for about ten days prevents the risk of phlebitis, and a compression garment is worn day and night for a month. Key point: you must avoid sitting directly on your buttocks for about ten days (sit on the back of the thighs with a cushion) and favour standing or lying on your stomach, until the grafts develop a blood supply. Allow about 2 weeks off work and a return to sport at around 6 weeks. The result is permanent by around 1 month: about 70% of the re-injected fat survives and lasts long-term, provided you keep a stable weight.

Am I a good candidate for buttock lipofilling?+

On average 60 to 70% of the re-injected fat develops a blood supply and persists; the rest is reabsorbed in the first few weeks. This is why the volume injected accounts for this reabsorption.
Is the BBL reimbursed?+
No: as an aesthetic procedure, buttock lipofilling is not covered by the French national health insurance.
What complications are specific to buttock lipofilling?+
Besides the common risks (haematoma, infection, phlebitis), lipofilling carries two specific risks: fat necrosis (breakdown of injected fat, forming a nodule that sometimes has to be drained) and, above all, fat embolism if fat is injected into a large gluteal vein — exactly what the rule of strictly subcutaneous injection by a qualified plastic surgeon prevents. An asymmetrical result may also warrant a touch-up.
What if the skin on the buttocks is loose?+
Lipofilling adds volume but does not tighten stretched skin. After significant weight loss with excess skin, a buttock lift / lower bodylift should be considered first (or at the same time); lipofilling then comes in to add fullness.
